“for real”という表現には、
「本当に、実際に」「本気で、真剣に」
などという意味があります。
ですから、“Is it for real?”と言うと、
「それって本当なの？」「それって本気なの？」「それってマジ？」
などという意味になります。
相手から聞いた話が本当なのかどうか判断できずに、
思わず相手に確認してしまうという場合に用いられる表現ですね。
“Is this for real?”と言うこともありますね。

のセリフで用いられています。

と言うLeoに対して、Margaretが、

と言っていますね。
